To meet the financial challenges facing the NHS and to for delivering it, requires organisations across the health and social care sectors to work in an integrated way.
Community services are often ‘the glue’ which joins primary care to secondary care, and also provides the critical link across the health and social care sectors. Unless these services can be delivered in a truly integrated way, gaps appear for patients and expensive duplication occurs between organisations.
Virgin Care has developed a range of community based pathway services which bring together a wide range of organisations to provide those who use our services with seamless care that removes duplication and is therefore more cost effective for commissioners.
Examples of these include:
MSK service, Hampshire
Operating in partnership with two acute providers and providing a comprehensive community based service involving GPs, consultants, extended scope physiotherapists, community physiotherapy, podiatry and diagnostics within the pathway.
Dermatology service, Reading
Provided in partnership with the Royal Berkshire Foundation Trust and offering a range of full dermatological procedures including community based assessment and treatment through to hospital based services.
Sexual Health Teesside
A comprehensive community based sexual health service provided by rationalising the number of service providers from four trusts into one organisation to deliver the service across four Primary Care Trusts.
