In Bath and North East Somerset, we were chosen by the local NHS and the local council to deliver the ‘Your care, your way’ programme following a two-year-long engagement programme defining how local people wanted services to be delivered and to run.

As part of the contract, we directly provide some services in the area and commission other organisations – including NHS, social enterprise and voluntary sector – to deliver services under the ‘Bath and North East Somerset Community Health and Care Services’ banner. We directly provide services for adults and children looking after the physical and mental health and wellbeing of everyone in the area through our community services.

We’ve been delivering health and social care services since 2006, and in that time we’ve seen and treated more than 5 million people across England on behalf of the NHS.

Our aim is to provide a joined-up service, to make sure you have all of the information you need and to make sure that you only need to tell your story once. By providing the right service to you at the right time we want to ensure that you feel supported and empowered to make informed choices.

We will achieve this by investing in technology systems which join up all of the information held about you and make it available to clinicians, with your consent, as well as directly to you – and all in a secure way – so you can be treated as a whole person and make informed choices.

  • Responsive to you – we ask everyone using our service and their families for feedback on their experience so we can find out what we’re doing well and what we need to do better. We have a network of children and young people’s service leads to share examples of best practice and learn together.
  • Safe and consistent – we have to meet rigorous Care Quality Commission (CQC) standards. We run our own internal inspections and have robust governance and reporting systems to monitor how all of our services are doing, so you can be confident that your care, safety and privacy are our absolute priorities.
  • Improving your experience – we’re always looking to improve your experience, whether that means working with GPs who refer into our services to improve this process so we see you quicker or providing unexpected magic touches. In our community hospitals we have introduced patient packs and handbooks to make sure all our guests have all the little essentials and information they need during their stay.
  • Care in the most appropriate place – we want to provide care in the best place for you, which is often in your own home. Through initiatives like our virtual wards in Surrey, where we work closely with our care partners (such as large hospitals), we can allow people with long-term conditions to be discharged from hospital and, wherever possible, supported at home.
