Some children and young people have complex health care needs and may require individual packages of continuing care to meet these needs. The Continuing Care Team (CCT) offer skilled nursing advice and support to enable technology dependent children to be discharged from hospital and to live at home.
The team supports families who have a child with complex healthcare needs, who are assessed to require additional health care support to enable their child to be cared for at home. Eligibility to receive continuing care is assessed using The National Framework for Children and Young People’s Continuing Care. DoH 2010.

The Continuing Care Team (CCT) offer support with discharge planning and assessment, alongside the co-ordination and planning of care packages.
Initially families will be offered an assessment to determine whether or not they are eligible for a care package (a care package is an individualised, plan of health care support, agreed by a number of professionals according to assessed need). For children who meet the continuing care assessment criteria, a individualised package of care is designed by the nursing team, based on information provided in the assessment. Support Workers are then recruited and trained to meet the specific, individual needs of the children; in order to support families in caring for their children at home. Care is provided for an agreed number of hours across the week and may include care at night or in the day, depending on need.
Depending on the child’s assessed needs and the agreed care package the support workers can work in a variety of settings e.g. home, educational settings.
