Welcome to the B&NES Heart Failure Service

We are a community heart failure service led by a team of heart failure specialist nurses within Bath and North East Somerset. The Heart Failure team are based at The Hollies, Midsomer Norton and are operational Monday-Friday 9am – 5pm, excluding weekends and bank holidays.

More about us

Through holistic assessment, the heart failure nurse specialists provide support, education and promote self -management strategies for those who have a confirmed diagnosis of Heart Failure, in collaboration families and their carers.  The service offer clinic appointments to those with a new and existing diagnosis of Heart Failure, and home visits for those who are housebound.

The Community Heart Failure Service delivers a diagnostic Echocardiograms clinic Supported by a Cardiac Physiologist from the RUH. Echocardiogram Clinics are carried out at Paulton Hospital, Keynsham Health Centre, Riverside Health Centre in Bath and the RUH

Referral to the community heart failure service is done through your GP who will have run a series of investigations prior to the referral, such as blood test and ECG.
