The service offers preventative and general care for patients with medical conditions. It also covers wound care, nail surgery and musculoskeletal disorders. Please note that we do not provide general nail care, fungal nail or verucca treatments.

How do I access this service?
If you are a patient in the B&NES area:
You can self-refer using the downloadable application form on the right hand side of this page (paper copies are also available at GP surgeries). Please note that if you are housebound we can offer you home visits, however you will need a GP referral.
