We provide continuing healthcare for children with nursing and medical needs who need additional support at home and in the community.

We support children and families with trained healthcare assistants who are managed and governed by a team of registered children’s nurses.
We work in partnership with families as well as community and hospital professionals to make sure that your child’s social and emotional needs are met. We support in educational settings alongside their healthcare needs.
It is important to us that children and families contribute to the development and improvement of services by providing feedback and participating in service evaluations. We value your perspective and if you are able, we would appreciate your assistance with the selection of new staff. We will also ask you to help us develop a care plan to individualise your child’s care and review this at regular intervals
We provide training to ensure our staff can competently provide care to children with clinical needs such as seizures; enteral feeding; oral and nasal pharyngeal suctioning; oxygen dependence; complex neurodisability; tracheostomy and ventilator care. In addition to meeting the physical; developmental and emotional needs of your child.

The Care Quality Commission (CQC) is the Government’s health watchdog, responsible for ensuring that healthcare services registered with it provide high quality care that is safe, effective and compassionate. The CQC inspected our service in June 2017 and rated it as ‘good’. View CQC report.

Where do I go for my child’s appointment?
We will let you know about the specific place and time of your child’s appointment when the appointment is confirmed.
If you need to bring any other children with you, please arrange for someone to supervise them during the appointment.
What happens during my child’s appointment?
Depending on your child’s needs, what happens in the home visit may vary.
How long will the appointment take?
Each visit is dependent on your child’s needs so appointment times will vary.
We would like to visit you at home to ensure care delivered continues to meet your child’s needs by:-
- Assess your child’s health
- Discussing care with you
- Introduce new staff
- To assist with the training of new staff
- Regularly check the clinical competencies of the healthcare assistants providing your child’s care
- Regularly check the home care file and your child’s medical equipment
Bringing someone with you to your child’s appointment
You are welcome to bring someone with you to your child’s appointment. Sometimes our staff may also request to have someone present for an appointment. If you or the member of staff is uncomfortable with the person, we’ll reschedule to a later date when the correct person is available
Arranging home visits
Your CHC nurse or the team manager will arrange home visits with you at your convenience.
Out of hours care
Complex healthcare is delivered 24 hours per day / seven days a week.
How do I cancel or change my child’s appointment?
If you need to cancel care please give as much notice as possible to enable us to allocate the staff to other care packages.
How does the service work?
1. Provision is determined through a continuing healthcare assessment of your child / young person’s needs by an Integrated Children’s Service Lead Professional
2. Following the assessment your Lead Professional will inform you of your allocation.
3. There after a member of the Complex Healthcare Team will meet with you to develop an individualised care plan to meet your child’s needs.
The services that we provide include:
- Support and assistance to children with complex healthcare needs and their families
- Partnerships with a range of professionals to make sure you child’s care is constantly updated to meet their needs and make sure that the family’s and consultant’s requirements for care are followed
- The same staff to work with your child to promote confidence and familiarity and
- A flexible service that will support the healthcare needs of your child as indicated and agreed through the assessment
How soon will my child be seen?
Following referral, we will ensure that we see your child as soon as possible. While we can’t be specific, we will contact you to discuss when we can see your child.
How will I get feedback about my child?
Keeping you involved in your child’s care is important to us. We’ll keep you updated on your child’s progress either face-to-face or in writing.
Who will treat my child?
Our team is dedicated to providing every child with the best care we can and every clinician has undertaken specific training. Our team are fully trained and are given access to the most appropriate training to enable them to focus on the individual needs of the young people and families they support.
Every member of our team has to provide evidence of their registration, training and criminal records status before they can work with your child.
