Welcome to East Staffordshire Learning Disability Primary Care Service

A Learning Disability Specialist Nurse provides support to primary care services to meet the needs of people with learning disabilities in East Staffordshire. This includes advice, training and support to identify barriers to accessing health and identifying what reasonable adjustments are required to improve access to health and improve health outcomes. The service also provides support for admission and discharge to Burton Hospital and works closely with their safeguarding team to raise standards in relation to care of their patients with learning disabilities.

The specialist nurse case manages patients with learning disabilities and complex needs who have problems accessing health services and provides advice on where to refer to other specialist services. They work closely with other local learning disability services i.e. Social Services – Independent Futures, South Staffordshire and Shropshire Healthcare NHS Foundation Trust (SSSFT) Learning Disability and Mental Health Services and Staffordshire and Stoke-on-Trent Partnership NHS Trust (SSOTP) primary care learning disabilities nurses.

Referrals can be made via the Care Coordination Centre (VCL.EastStaffsCCC@nhs.net / 0300 323 0930).

The referrer will be contacted by the specialist nurse to inform of the outcome of their referral.
