Accessing your records
From 1 July 2015 this service is no longer commissioned.
You should contact our Information Governance Team if you wish to access medical records relating to care provided by us.
Making a complaint
As always, the speediest way to resolve a complaint may be to speak to the people involved in your care but if your complaint related to the service provided to you before 1 July 2015 then please contact our Customer Services Team on 0300 303 9509.
